ABSTRACT

PDAM Sragen city is one of the regional drinking water companies that conducts a customer site survey process for the initial process of installing water pipes. The agency has problems in terms of the data collection process that is still being carried out by writing in a notebook brought by the field survey officer. This causes several problems, including data that is easily lost and damaged because the storage media still uses notebooks. In addition, the absence of preparing a customer location survey report is a problem that must be resolved by the agency. Based on these problems, the formulation of the problem that the author discusses is how to design and create an information system for collecting data on customer location survey results. In this study, the author limited the problem only to the process of collecting the results of field surveys of potential customers in PDAM Sragen city. Enter data on the information system is user data, customer data and survey data. The data output in the information system is a data report on the results of a field survey. The information system that the author designed is web-based using PHP programming language and MySQL databases. The purpose of this study is to design and create an information system for collecting data on field survey results at PDAM Sragen city and be able to produce an information system that can help the administration in managing data on field survey results
